Certificate of Service — All Ontario Forums

A Certificate of Service is the licensee-signed proof of service. No commissioner is required: the lawyer or paralegal certifies on their own signature that the documents were served. Every Ontario civil and family forum has its own form number.

RCP Form 16B.1 (Superior Court, Rule 16.09(1.1))

Family Form 6C

Frequently asked questions

Why use a Certificate instead of an Affidavit?
Saves a trip to a commissioner. A licensee signs the Certificate on their own authority under the relevant rule; there is no need to swear in front of someone else. Both are accepted by the court.
Who is a "licensee" for these forms?
A lawyer (Class L1 or L2) or a paralegal (Class P1) licensed and in good standing with the Law Society of Ontario. The licence number prints on the certificate.
