POA Court handles tickets and charges under provincial statutes — Highway Traffic Act, Liquor Licence Act, by-laws, and other regulatory offences. The forms come from Reg. 950 under the Provincial Offences Act. Defendants and their representatives use them to elect trial, file appeals, and bring motions.

Notice of Motion + General Form of Affidavit (POA 0006 CSD). Used for reopening applications under s. 11, bail-pending-appeal under s. 110, extension of time to appeal under s. 116, stay-of-conviction motions, and Court of Appeal leave applications.
Written acknowledgment from a client that they understand the consequences of pleading guilty — voluntariness, the Anthony-Cook test for joint submissions, MTO consequences, that sentence rests with the JP, and that they waive trial. Sign before the plea is entered to protect against later "ineffective assistance" allegations.
