Bilingual one-page form. A Part I defendant files this with the court office to put the ticket on the trial list instead of paying. Indicates whether the officer is required to attend and the language of trial.

Ontario Provincial Offences Act (POA) Court Forms
POA Court handles tickets and charges under provincial statutes — Highway Traffic Act, Liquor Licence Act, by-laws, and other regulatory offences. The forms come from Reg. 950 under the Provincial Offences Act. Defendants and their representatives use them to elect trial, file appeals, and bring motions.
Replacement of Missing Documentation (POA 0831)
Used when the defendant's original yellow ticket is lost. Combines the replacement-document request with a one-of-three election: plea+payment, plea+explanation in court, or trial.

Plea Comprehension
Written acknowledgment from a client that they understand the consequences of pleading guilty — voluntariness, the Anthony-Cook test for joint submissions, MTO consequences, that sentence rests with the JP, and that they waive trial. Sign before the plea is entered to protect against later "ineffective assistance" allegations.
